List of Requirements for Philippine Passport:

- Personal Appearance: You must appear in person at the designated passport application center. This is necessary for identity verification purposes.
- Accomplished Application Form: Fill out the application form completely and accurately. This form will capture your personal information and other relevant details.
- Original and Photocopy of PSA Authenticated Birth Certificate on Security Paper: Present your original birth certificate issued by the Philippine Statistics Authority (PSA). Ensure that it is authenticated and printed on security paper. Also, bring a photocopy of the same document.
- Additional Requirement for Married Females: If you are a married female and using your spouse’s last name, you must also provide the original and photocopy of the PSA Authenticated Marriage Contract on Security Paper or Report of Marriage. However, if you choose to retain your maiden name, a marriage contract is not required.
- Local Civil Registrar Copy (if applicable): If your PSA birth certificate is unclear or cannot be read, you must obtain a Local Civil Registrar Copy to supplement the required document.

SUPPORTING DOCUMENTS FOR ADULT NEW APPLICATIONS

- Late Registered Birth Certificate:
- If your birth certificate was registered at least ten (10) years ago, no additional supporting documents are needed.
- If your birth certificate was registered less than ten (10) years ago, you must submit IDs that pre-date the late registration or current IDs with NBI Clearance.
- No Birth Certificate or Report of Birth:
- If you were born after 1950 and have no birth certificate or report of birth, you must file for late registration with the Local Civil Registrar (LCR) or Consular Office. Afterward, submit the original copy of the PSA-authenticated late registered birth certificate along with IDs that pre-date the late registration or current IDs with NBI Clearance.
- If you were born on or before 1950 and have no birth certificate, submit the original and photocopy of the PSA Authenticated Certificate of No Birth Record. Additionally, provide an Affidavit of Two Disinterested Persons attesting to your identity. This affidavit should include photocopies of the IDs of the two disinterested persons mentioned in the document.
- Reverting to Maiden Surname:
- If you have undergone a marriage dissolution and want to revert to your maiden surname, submit the original and photocopy of your PSA birth certificate and acceptable IDs.
- Additionally, provide the original and photocopy of the annotated PSA Marriage Certificate (MC) or Report of Marriage (ROM) stating that the marriage has been dissolved.
- Lacking Data in Birth Certificate or Report of Birth:
- If your birth certificate or report of birth has missing data, present the original and photocopy of the proof of filing of a supplemental report from the Local Civil Registrar (LCR).
- A discrepancy in Data:
- In case of a discrepancy in the first name on your birth certificate, submit the original and photocopy of the petition and supporting documents for correction of the first name filed with the Local Civil Registrar (LCR) or consulate.
- If the discrepancy involves other data, provide an annotated birth certificate authenticated by the PSA reflecting the corrected entry.
- Dual Citizen:
- Schedule a confirmed online appointment and provide government-issued IDs. Refer to the list of valid Philippine Government Issued IDs or their secondary citizenship/residence counterparts.
- If you possess dual citizenship from birth, submit the original and photocopy of the PSA-authenticated Birth Certificate/Report of Birth.
- If you reacquired or retained Filipino citizenship as specified in RA 9225, provide the original and photocopy of the identification certificate issued by a Philippine Foreign Service Post (FSP) or the Bureau of Immigration (BI).
- Naturalized Filipino Citizen:
- Submit the original and photocopy of the Identification Certificate of Naturalization issued by the Bureau of Immigration (BI).
- Filipino Citizenship Obtained by Election:
- Provide the original and photocopy of the Affidavit of Election of Philippine Citizenship.
- Also, include the original and photocopy of the Identification Certificate of Election issued by the Bureau of Immigration (BI).
- Citizenship Granted by Act of Legislation:
- Submit a certified true copy of the law granting citizenship.
- Additionally, provide a foreign birth certificate authenticated by a Philippine Foreign Service Post (FSP).

- Accomplished Application Form: Complete the application form with all the necessary information regarding the minor applicant.
- Personal Appearance of Minor Applicant and Either Parent or Authorized Adult Companion: Both the minor applicant and either one of the parents or an authorized adult companion must be present during the application process.
- Philippine Statistics Authority (PSA) Birth Certificate: Present the original PSA-issued birth certificate of the minor applicant.
- Local Civil Registrar Copy (if applicable): If the PSA birth certificate is unclear or cannot be read, you may be asked to provide a Local Civil Registrar Copy as a supplementary document.
